Comprehensive, holistic treatment is based on a personalized plan that changes with the evolving needs of each patient and family. The goal is to enhance the quality of life of terminally ill patients and provide emotional and spiritual support to patients and their families. The patient’s physician guides the interdisciplinary care team. Members include skilled nurses, home health aides, social workers, physical therapists, occupational therapists, dietitians, spiritual care providers and trained volunteers.
